3. Referrals from hospitals or other centers/ professionals must be accompanied by a letter of reference and a doctor's or professional's report.

* where necessary, the school has the right to send the child for a professional assessment. The fees will be borne by the parents.

1st appointment:
1. Screening/ interview by Principal and Head of PG dept.
a. Every Monday
2. Audiological Assessment Part 1 will be carried out.
a. Initial fee of $120 for a package of a maximum of 3 hearing assessment, including the recommendation and fitting of hearing aids.

Subsequent appointments:
1. Parent Guidance session commences 2 weeks after initial hearing aid
fitting (refer to PG first PG and 1st info protocol attached).
2. Follow-up Audiological reviews.

School enrolment into Preschool/ Primary school:
1. Approximately 4-6 PG sessions (general guideline which may vary with
each families needs and commitment shown to school programme) before
enrolment into school for Nursery, Kindergarten or Primary school can be
discussed
2. One meeting or Parent Guidance session with head of Preschool or
Primary school before enrolment.
3. Date of 1st day in school confirmed.
